For orders placed by mail, you can download a copy of my printable order form using the link below. Use the printable order form when:
- You want to pay by check or money order.
- You want to pay by credit card but simply prefer ordering by mail to using the online shopping cart.
To view the form, you'll need a PDF reader such as the Adobe Acrobat Reader. You probably already have it, but if you don't you can download it free at Adobe's web site. Another free PDF reader that we highly recommend is the Foxit Reader.
